How it works

It’s Easy to Join


  1. Fill out Member Form below and press the "Submit" button.
  2. An invoice will be emailed to pay online, or you can pay in the office.




  1. New members pay $150 for the year.
  2. Existing members pay $75 yearly.
  3. If you test at a facility other than the Key Largo office,
  4. there will be an additional charge of $20
  5. Membership dues go towards lab fees, MRO fees, correspondence fees and other.
  6. Membership covers initial testing, random tests throughout the year, correspondence for proof of membership and general guidance to navigate the USCG compliance requirements.
  7. Membership does not cover post-accident testing, saliva alcohol swabs, postage or substantial staffing hours dedicated to a single members correspondence. (Saliva alcohol swabs can be requested from our office. They are not sold for a profit)



Directions


  1. Please read and complete each part of the membership form below.
  2. Once you fill out this form, we will receive your information via email and follow up with you for payment and testing, if necessary.
  3. Note: If you only need to make a payment, contact us by email at customerservice@keysmobilemedical.com and we will send you an invoice to pay online.
  4. A pre-employment drug screen is required for any person joining a drug testing program unless he/she has been tested within the past 6 months or has been subject to random testing for at least 60 days within the past 6 months.
  5. If you are not required to take a pre-employment test, please enclose original documentation with application.
  6. If you need a drug screen we will provide you with a collection site in your area, please fill out the application accordingly.



Membership Agreement

The Keys Consortium will provide each member:


  1. A specimen collection site near the applicant’s residence.
  2. Computer generated random selection.
  3. Random notification by mail, email or telephone
  4. Medical Review Officer (MRO) services.
  5. Help with a Drug free workplace policy.
  6. Record keeping of all positive test results for five years, and negative results for one year.
  7. A membership letter
  8. Information on substance abuse and counselor referral service.
  9. Verification confirming program compliance for employers, licensing renewals, and upgrades.



New membership or Renewal of your membership shall be deemed as consenting to the following agreement.

Members must:


  1. Properly follow random testing instructions.
  2. Promptly notify Keys Consortium of any change in mailing or email address, phone number, employer change, or membership status.
  3. Follow guidance provided by Keys Consortium.
  4. Members shall not hold Keys Consortium liable arising out of or in connection with a member’s failure to comply with DOT policy or breech of this agreement.
  5. Failure to comply with provisions of this agreement will result in Coast Guard notification and possible termination from program.



To submit this form is to agree and acknowledge that I have read the membership agreement, that I fully understand that violation of this policy will be grounds for Coast Guard notification and possible termination of my membership.


US Coast Guard Requirements

Company Policy Letter

Each firm must have a written policy on drug and alcohol use. This must be signed by all members and a copy must posted. We will supply you with a compliant policy.

Employee Training

1 hour of training on drug and alcohol use/abuse for each employee is required. We provide the necessary video and written materials with paid membership.

Medical Review Officer

All results must be reviewed by a certified MRO. The Keys Consortium provides a MRO that reviews each test result. In case of a positive the MRO will interview the donor by telephone to determine if there is a valid reason for the results

Record Keeping

Records must be kept for all test results, negatives for 1 year, positives for 5 years. Keys Consortium maintains all test results for you. We also complete the annual MIS form for each client in good standing.

SAMHSA Certified Lab

Testing must be done by a lab that meets DOT/HRS standards. Keys Consortium tests are done in strict accordance with the DOT/HRS by certified collection site technicians and processed by a SAMHSA certified lab

Pre-Employeement Testing

A negative test result must be provided prior to membership into any drug testing program. A test is not necessary if proof of a negative result is provided from within 6 months of membership date with no positive result during that 6 month period or during the previous 185 days proof of membership in another compliant drug program in good standing for at least 60 days

Random Testing

All members are required to be randomly tested at a rate of 50% of testing pool. Selection must be statistically valid so that there is an equal chance for any member to be selected. Keys Consortium administers a valid program for all members. All random tests are covered with membership, regardless of number of times a member is selected. .

Reasonable Cause Testing

If the employer believes an employee is using an illegal drug they must arrange for a specimen to be collected. We will advise members of what constitutes reasonable cause and perform testing if necessary.

Periodic Testing

Periodic tests must be done for license issue, license renewals and upgrades. Members enrolled over 60 days are exempt from periodic testing. We will provide you with a letter to give to the Coast Guard upon renewal .

Serious Marine Incident Testing

The marine employer shall ensure that all persons directly involved in a serious marine incident are tested. Keys Consortium makes every attempt to be available for serious marine incidents and post-accident testing.
