Hormones govern nearly every aspect of how we feel: our energy, our mood, our sleep, our libido, our body composition, our cognitive clarity, and even the quality of our skin. When hormones shift, whether through menopause, perimenopause, aging, or other factors, the effects can touch every corner of daily life in ways that are often dismissed, misattributed, or simply accepted as inevitable.
At Sanger Avenue Medical Aesthetics, Dr. Patricia Wilcox, M.D. (board certified in Obesity Medicine) offers Biote® bioidentical hormone pelleting as a clinically supported, personalized approach to hormone optimization. Biote uses bioidentical hormones (compounds molecularly identical to the hormones your body naturally produces) rather than synthetic alternatives, and delivers them through small pellets inserted subcutaneously for steady, consistent hormone release over time.
The result, for many patients, is a noticeable improvement in the symptoms and quality of life concerns that declining hormones can produce, without the peaks and valleys of other delivery methods.
Alongside hormone pelleting, we also offer Biote® nutraceuticals: a line of physician-grade supplement formulations designed to support and extend the benefits of hormone optimization.
Bioidentical vs. Synthetic Hormones: Synthetic hormones used in conventional hormone replacement therapy are chemically similar to but not molecularly identical to the hormones your body produces naturally.
Bioidentical hormones, as used in Biote pelleting, are molecularly identical to endogenous hormones. This means they interact with hormone receptors in the same way your body's own hormones do. Many patients and providers prefer bioidentical hormones for this reason, and Biote's pellet delivery method provides an additional advantage: consistent, steady hormone release that avoids the fluctuating levels associated with pills, patches, creams, and injections.
Pellets vs. Other Delivery Methods: Hormone replacement can be delivered through pills, patches, creams, injections, or pellets; and each delivery method has different characteristics.
Pills are processed through the digestive system, which can affect consistency. Patches and creams can produce variable absorption depending on skin condition and application site. Injections produce peaks and troughs as hormone levels rise after injection and then drop before the next dose.
Pellets release hormones steadily over 3-6 months, producing consistent hormone levels without peaks or troughs, and require no daily action from the patient. Many patients find that this consistency translates directly into more stable mood, energy, and symptom relief.
Biote addresses common symptoms of hormone imbalance including:
Biote hormone pelleting is a brief, straightforward in-office procedure that takes approximately 15 minutes.
Before your first pellet insertion, Dr. Wilcox will order a comprehensive hormone panel through lab testing to establish your current hormone levels and identify specific imbalances. Your pellet formulation is then customized based on your lab results, your symptoms, and your individual health history, not a one-size-fits-all dose.
The pellets themselves are small (roughly the size of a grain of rice) and are inserted subcutaneously through a tiny incision, most commonly in the upper buttock area. A small bandage covers the insertion site. The procedure requires no stitches and takes minutes. Most patients return to normal activities the same day, though strenuous lower body exercise is typically avoided for a short period after insertion.
Most patients require re-pelleting every 3-6 months as the pellets are gradually and completely absorbed: women typically every 3-4 months, men every 5-6 months.

Hormone optimization doesn't end with the pellet, and results can be meaningfully enhanced with the usage of the Biote line of nutraceuticals.
We mainly carry DIM SGS+ in-store as we find it is a significant addition to any pelleting patient's supplementation lineup. However, DIM SGS+ as well as Biote's other nutraceutical offerings are available at the link below, and overviews of their different nutraceutical options can be found below the Frequently Asked Questions section of this page.

Bioidentical hormones are compounds that are molecularly identical to the hormones your body naturally produces (including estradiol, testosterone, and progesterone). Unlike synthetic hormones used in conventional hormone replacement therapy, which are chemically similar but not molecularly identical, bioidentical hormones interact with your body's hormone receptors in exactly the same way as endogenous hormones. Biote bioidentical hormones are derived from plant sources and compounded to match your body's specific hormonal needs based on lab testing.
Small pellets (roughly the size of a grain of rice) containing a customized dose of bioidentical hormones are inserted subcutaneously through a tiny incision, most commonly in the upper buttock area. The pellets release hormones steadily as they are gradually absorbed by the body over 3-6 months. This steady release maintains consistent hormone levels without the peaks and troughs associated with other delivery methods. The pellets are completely absorbed (nothing is removed later) and re-pelleting is performed before levels drop back to symptomatic ranges.
Conventional HRT typically uses synthetic hormones delivered through pills, patches, or creams. Biote is different in two main ways. First, it uses bioidentical hormones instead of synthetic ones. Second, it delivers them through pellets that provide steady release over months instead of the daily ups and downs of pills or creams. Many patients who tried conventional HRT and weren't happy with it find that Biote's steady delivery and bioidentical formula feels like a completely different experience.
Before your first Biote treatment, Dr. Wilcox will order a full hormone panel to check your current levels of estradiol, testosterone, and other relevant hormones. Your pellet dose is then built specifically around your lab results, your symptoms, and your health history. This personalization is what makes Biote work well. Your dose isn't generic, it's based on what your body actually needs.
Most patients feel very little discomfort. The insertion itself only takes a few minutes, and most patients say the whole thing is much easier than they expected. Some mild bruising or tenderness at the site is normal in the days after and clears up on its own.
Many patients notice early improvement in energy, mood, and sleep within 2-4 weeks of their first insertion. Bigger improvements in libido, body composition, mental clarity, and other symptoms usually build over the following weeks as hormone levels settle into an optimal range. Since everyone's starting point and symptoms are different, your timeline will look a little different too. Your provider will help set realistic expectations at your consultation.
Women typically need re-pelleting every 3-4 months. Men typically need it every 5-6 months. Most patients settle into a rhythm with their pelleting schedule pretty quickly and treat it as a regular wellness appointment.
Biote hormone optimization is generally well tolerated. Some patients notice temporary side effects during the initial adjustment period as hormone levels settle, and these vary depending on the individual and which hormones are being optimized. Your provider will go over potential side effects relevant to your specific plan at your consultation. Like any medical treatment, Biote isn't right for everyone, and patients with certain hormone-sensitive conditions or medical histories may not be candidates.
Yes! Hormonal changes in women get talked about more, but men go through significant hormonal shifts with age too, especially declining testosterone, which affects energy, muscle mass, libido, mood, focus, and overall wellbeing. Biote offers hormone optimization for both men and women. If you're a man dealing with symptoms of hormonal decline, we'd encourage you to schedule a consultation with Dr. Wilcox to talk about whether Biote makes sense for you.
Biote Hormone Pelleting addresses the hormonal environment behind a lot of the changes patients come to us for, and balancing hormones can support and extend results from nearly every other service we offer. For gynecologic and pelvic health concerns, Biote pairs naturally with MonaLisa Touch, FormaV, or vTone, since balanced hormones support tissue quality and help maintain the results of those laser and RF treatments over time. For aesthetic concerns, balanced hormones support skin quality, elasticity, and overall health, so patients getting Morpheus8, SkinPen, injectables, or resurfacing treatments (like NouvaDERM or CO₂ resurfacing) may see results that hold up better and last longer when their hormones are dialed in. Medical-grade skincare from SkinCeuticals or Hydrinity can support skin health from the outside while Biote supports it from within. Your provider will talk through how hormone optimization fits into your overall wellness and aesthetic goals at your consultation.
