After booking and paying deposit, You will complete your consent forms and GFE using the buttons bellow.

Please complete all required forms, even if you are only receiving one service at your upcoming appointment. This ensures the appropriate documentation is already on file should you return for future treatments such as lip filler or neurotoxin (Botox, Dysport, Xeomin, or Daxxify).

All required consents and the GFE must be completed prior to your appointment.

We understand that things come up; however, last-minute cancellations and no-shows significantly impact our small business and prevent us from offering that appointment time to another client. We kindly ask that you provide at least 24 hours’ notice if you need to cancel or reschedule.

Cancellations made with less than 24 hours’ notice, as well as no-shows, will be subject to a fee equal to 50% of the scheduled service total.

This fee must be paid before booking a future appointment.
