-An Additional $20 is to be charged as the travel fee.
-Intake forms need to be filled out prior to the appointment. If there is no intake form on file or proper communications regarding not receiving an intake form, a No Show fee will be charged before rescheduling. No Show and Cancellation Fee is $50. This would need to be paid before the next appointment is scheduled.
